LCHS Girls Soccer Team Rules and Philosophy Acceptance Form

My parent(s) and I have read and understand Coach Coffey's Coaching Philosophy and the Little Chute High School Girls Soccer Team Rules. We understand that they are specific to the Girls Soccer Team, and that the Little Chute High School Extra-Curricular Guidelines fully and clearly state the participant responsibilities and the Little Chute School District's expectations for participating in the district's extra-curricular programs.
